Dear Sir or Madam
When preparing the enclosed bottles, please follow the directions below:
- When preparing your sampling kits, fill out the form to include the number that’s on the sample bottle along with the information from the corresponding transformer that you pulled that sample from. This would include the estimated gallons and or pounds, dimensions if the unit is >6’ tall and an out of service date. Repeat this for each individual unit/sample.
- On the attached chain of custody, please fill out all area’s that apply to you and the samples you are sending. It’s imperative to include the actual sampler’s name on the chain of custody.
- Place the oil filled bottles in the mailers.
- Tape both sides of the mailer for security.
- Insert the mailer into the plastic bag before placing into the return mailer. (You probably can send them back to us in the same box that they were mailed to you in.)
- Please fax or email us a copy of your manifest information sheet along with your completed chain of custody. On these documents please include when you are sending them to our lab so we can make them aware of their arrival.
- Please use the return lab address information that TCI has provided.
It’s very important to completely fill out the manifest information sheet and chain of custody and fax them to TCI NY prior to including them with your samples heanding to the lab.
